If you want an appointment you can do either of the following:

• Visit your GP or Specilalist and have them send a referral through to the Wadestown Allergy Clinic via email, and

​

• Fill out the Wadestown Allergy Clinic Questionnaire (pdf, below) and send it back to us either in the mail or via email or in person. We will be in touch with you to let you know if we are able to help you with your needs and then we can organise an appointment time and date with you.
